To My Patients:

After almost thirty years of practicing pediatric ophthalmology, I am retiring from clinical practice and will close my Rockwall office on October 16, 2008.  The office phone number and web site will remain in service after this date for any questions you may have regarding referral to other physicians or medical records.  For your convenience, I have posted a list of other pediatric ophthalmologists in the DFW area.  Please click the "other ophthalmologists" tab to view the list.

If you would like a copy of medical records, please send the following information:

1)      Signed and dated request for medical record

2)      Patient(s) name and date of birth

3)      Name of person making the request and relationship of that person to the patient

4)      Contact information – mailing address, phone numbers, email.

5)      While we are not making a copying charge for your records, we do ask that you enclose a large envelope (at least 9x12) stamped with $1.00 postage.
